Brief Description

Prompt Healthcare Staffing are looking for a new Care Coordinator Apprentice. You will be studying toward your Adult Care Worker apprenticeship Level 2. Due to requirements of the role a full UK driving licence is mandatory.

Vacancy Detail

To provide high-quality, person-centred care and support to individuals in line with their care plans, promoting independence, dignity, choice, and well-being while ensuring their safety at all times. The role involves supporting individuals with daily living activities and contributing to a positive, caring environment. To report to the office team observations in the community. To review care plans.  


Key Responsibilities: 



  • Deliver person-centered care tailored to each individual's needs and preferences

  • Assist with personal care, including washing, dressing, toileting, grooming, and oral hygiene

  • Support individuals with eating, drinking, meal preparation, and nutrition

  • Administer or assist with medication in accordance with company policies and training

  • Encourage and promote independence and life skills wherever possible

  • Support individuals to access the community, attend appointments, education, employment, or social activities

  • Maintain accurate, clear, and timely care of records and documentation

  • Monitor and report any changes in an individual's physical or mental health

  • Safeguard vulnerable adults and children by recognising and reporting concerns promptly

  • Follow infection prevention and control procedures, health and safety legislation, and moving and handling practices

  • Communicate effectively with service users, families, colleagues, and healthcare professionals

  • Respect confidentiality and comply with data protection requirements

  • Provide emotional support, reassurance, and companionship

  • Participate in team meetings, supervision sessions, and mandatory training

  • Promote equality, diversity, dignity, and respect in all aspects of care
